Perfume (also called Scented Water) is a required item in Like A Dragon: Ishin for a substory involving a lovelorn woman that you meet next to the ferryman outside of Teradaya.
After giving you 3 Ryo to buy some perfume with, you are sent off to Mukurogai to find the vendor who is selling the perfume. To find this vendor, you need to go to the same place where you can practice your cannon skill in the east of the island, then climb up the stairs.
Take a left (here’s where you can also bump into Creek again, the bumbling foreigner, to progress that substory) then approach the person handing something over to a customer. After a brief interaction, you will have to take part in a very straightforward fight against this mysterious man.
Once defeated, the mysterious man will then reveal himself as the person selling this perfume and will spritz you with it, then give you a bottle for free. It’s not long before the effects of the perfume drive him crazy too, leading into one of the wildest chase scenes in the whole series.
Evade his loving embrace and follow the marker all the way back to the lovelorn woman outside of Teraday to complete this substory and earn some Virtue. You also get to keep the 3 Ryo for yourself, which is pretty nice.
